Ketchum, Wood & Burgert Pathology Associates provides services that are requested by your
health care provider. Since you, the Patient, do not come to our facility
for treatment, we must rely on the billing information supplied by your health care
provider. The information is used by us to submit a claim, on your behalf, to your
insurance company.
To assist us in this process and so that you are not sent an incorrect billing statement,
please make sure to provide your health care provider with the following:
- complete and accurate names, dobs and addresses for both yourself and the individual
accepting payment responsibility (guarantor).
- complete insurance information, including the insurance company name and address and
your policy number
- a copy of your insurance card.
The information you supply to your health care provider will be sent to us along with your
test or specimen. This information, when accurate and complete will streamline our efforts
to satisfy your financial responsibility.
If you receive a billing statement from us that does not reflect your most current
information, please contact us as soon as possible so that we can correct the situation.
