For physician office visits, our services are requested by your physician and include processing, reviewing and rendering of a diagnosis for specimens submitted to Our laboratory.

As a courtesy, Ketchum, Wood & Burgert Pathology Associates files a claim to your insurance company. We work with your insurance company to provide all the necessary information to receive payment from them, on your behalf.

You have received a billing statement from us due to of one of the following reasons:

• As a courtesy, while we are working with your insurance company.
• We have finished working with your insurance company, and there is a balance due.
• Your insurance company has indicated that you have a co-payment, co-insurance or deductible that is your responsibility.
• Your insurance company has denied the claim made on your behalf and the balance is your responsibility.
• You have no insurance that we are aware of.
